ENTRY GUIDELINES

Sunday, July 19, 2009
Dear Parents and Students,

Please see the attached memorandum below on the Revised Rules of Entry to the PSHS Main Campus and Guidelines Re:A(H1N1). A REPLY SLIP needs to be signed by both the student and parent/guardian, upon return to school on July 27. An e-copy of document is attached herewith.

As precautionary measures against the Influenza A H1N1 virus, the following rules and guidelines shall be implemented effective July 18, 2009:
1. Don’t come to school if you are sick.
All school officials, students, employees, parents and visitors (everyone) shall submit themselves to the thermometer scan upon entry at the main gate and again at the entrance of the residence halls (for dormers and their 2 companions).
2. Those with fever and other ILI symptoms shall not be allowed inside the campus.
The school has only a one-bed clinic and has no capacity to deal with a large number of sick students.
3. Check-in time for dormers shall be at 2:00-8:00 p.m., Sunday only, to decongest the traffic at the gate early Monday.
4. Only a maximum of 2 companions for dormers shall be allowed inside the campus.
5. Parents waiting for their children are discouraged from mingling with students at the front lobby. Please fetch your children at about their dismissal time.
6. Students who develop ILI symptoms while inside the campus should immediately consult Dr. Corazon Batoon, school physician at the clinic. Your parents will be called to bring you home. Teachers and students should help see to it that anyone with ILI symptoms is brought to the clinic.
7. Students who are sick and who voluntarily stay at home shall be accorded utmost consideration by their teachers and are entitled to make-up for missed exams and requirements. However, they must present an excuse slip and be subject to the school physician’s examination.
8. Students who persist in coming to school even if they are sick shall be sent home and shall lose their right to make-up for missed exams and requirements.
9. Bring your hygiene kit everyday. It will be checked by your homeroom and subject teachers before the start of each class. The hygiene kit should contain: soap, alcohol, sanitizer, tissue and masks.
10. Wear your mask everyday and dispose of it properly.

FOR STRICT COMPLIANCE.
(sgd) DR. HELEN E. CAINTIC
CAMPUS DIRECTOR
